Inspector Notes
An unannounced monitoring inspection was initiated and completed on 5/22/2025 in response to a self-report received by the licensing office on 5/16/2025 relating to an injury of a child requiring outside medical treatment. The inspector reviewed one child?s record and two staff records on site.
This inspection included document review (i.e. attendance records for children and staff, staff training, staff background checks), tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
