Inspector Notes

An unannounced self-report investigation was initiated on 1/11/2024 due to a self-report complaint made to the Department of Health and Safety in the area of physical abuse. The investigation was completed on 01/24/2024. The inspector was on site on 1/11/2024 from 3:50pm until 5:00pm.

Two children's records and three staff records were reviewed. Interviews were conducted as well.

Information gathered during the inspection determined the complaint to be NOT VALID. Non-related violations were issued to the program. Violations are documented on the violation notice issued to the program.

Based on record review, the center failed to ensure each staff member has a documented TB test or screening every two years.

Evidence: Staff #3 was working day of inspection on 1/11/2024. Staff #3 had a TB screening dated 09/14/2021.
