Inspector Notes

An unannounced self-report investigation was initiated on 1/23/2024 due to a self-report complaint made to the Department of Health and Safety. The investigation was completed on 01/24/2024. The inspector was on site on 1/23/2024 from 2:45pm until 4:15pm.

Two children's records and two staff records were reviewed. Interviews with staff were conducted as well.

Information gathered during the inspection determined the complaint to be NOT VALID. Non-related violations were issued to the program. Violations are documented on the violation notice issued to the program.

REPEAT VIOLATION
Based record review, the center failed to ensure that the separate record for each enrolled child shall contain all of the elements as required by the standards.

Evidence:
The record for Child #2 did not contain the following information:
Previous school attended, Food allergy and chronic physical problems.
Plan of Correction: The PD will ensure that all information is in the child's file before enrolling. Parents have already completed and returned form.
