An unannounced self-report investigation was initiated on 5/1/2024 due to a self-report complaint made to the Department of Health and Safety. The investigation was completed on 5/1/2024. The inspector was on site on 5/1/2024 from 2:00pm until 2:35pm.
Two children's records and two staff records were reviewed. Interviews with staff were conducted as well.
Information gathered during the inspection determined the complaint to be NOT VALID. No violations were issued to the program.
