An unannounced monitoring inspection was initiated on 10/09/2024 and completed on 10/30/2024 in response to a self-report received by the licensing office on 10/08/2024 relating to forbidden action. The inspector reviewed 1 child?s records and 2 staff records on-site. This inspection included document review, and interviews.
