Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 07/18/2024 and completed on 07/18/2024, as a part of the biannual licensure period. The on-site inspection began at 12:10 PM and ended at 2:05 PM. The inspector reviewed compliance in the areas listed above. There were 23 children present and 4 staff.The inspector reviewed 5 children?s records and 4 staff records on-site 07/18/2024. This inspection included:

? document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms),
? tour of the facility,
? interviews, and
? observations

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/29/2024. A POC submitted after this date will not appear on the public website.

Based on observation and interview, the center did not ensure food from home container is clearly dated and labeled in a way that identifies the owner.

Evidence:
1. Staff 2 held up a water bottle and asked who it belonged to. When LI observed and interviewed staff, the water bottle was not labeled or dated to identify the owner.
