Inspector Notes

A monitoring inspection was initiated on 08/16/2023 and concluded on 08/16/2023 from 12:50 PM to 2:00 PM. There were 217 children present, ranging in ages from 5 years to 11 years, with 17 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of five children?s records and the five staff?s records were reviewed for updates.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on a review of three injury reports, the center failed to document all required information on one of the three reports reviewed.

Evidence:

The injury report for child 6 dated 8/6/23, did not contain the date, time & how the parent was notified.
Plan of Correction: 1. Facility Director has completed a "Sample" incident report and spoken directly to all staff reviewing properly completing all aspects of the injury report. Staff are then given their own report to complete with a fictitious scenario to ensure that they understand all sections must be completed.
2. During Summer Camp and full days of care, team members will bring incident reports immediately to the front desk upon completing report. The front desk staff will then email a copy of the incident report to parents/guardians and ask them to stop at check out to sign the report. After emailing the front desk member will, notate time and date of email, method of communicating incident as email and keep the report at front desk for evening check out.
2B. For Afterschool and part day care, ONLY leads, and admin will be completing incident reports. That staff member is then responsible for staying on site until the parent arrives and completes the incident report.
