Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was conducted on April 6, 2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 29 children present, ranging in ages from 4 to 7 years and in the care of 5 staff. The inspector reviewed compliance in the areas of administration, physical plant, staff qualifications, staffing and supervision, programming, special services, special care provisions and emergencies. A total of 3 children's records and 4 staff records were reviewed.

Information gathered during the inspection determined one non-compliance with applicable standards or law and the violation was documented on the violation notice issued to the program.

Based on the review of three children's records and verification from the director, there was not an immunization record in the file for one child showing that the child was adequately immunized.

Evidence
1. The record for Child #2 did not contain documentation that the child had received all of the immunizations required. Date of enrollment for Child #2 was 08/24/2021.
2. The director confirmed that the immunization record was not in the child's file.
Plan of Correction: The director will reach out to the parent of the child and obtain documentation of the child's immunization record as soon as possible.
