Inspector Notes

An unannounced monitoring inspection was conducted today from 11:38am-12:22pm with the provider on site. There were 7 children with 1 caregiver. A sample of 2 children's files and 3 caregivers/household member's files were reviewed. The children were engaged in the following: toy play. Interactions between the children and caregiver were positive. The physical space, attendance records, drill record and the Emergency Preparedness Plan were reviewed. Based on review of children's records, the provider did not obtain a physical examination and immunization record for each child that was signed by a licensed physician, the physician's designee, or an official of a local health department on the current form approved by the Virginia Department of Health or a physician's form and dated.
Evidence:
1.Child #1 and Child #2 (start date 6/27/2023) had an immunization record and physical, but it was not signed and dated by a physician.
Plan of Correction: I will ask the parent to get it.

Based on observation, the provider did not keep information contained in a child's record confidential.
Evidence:
1. The provider had the supplemental page posted from the 1/05/2023 inspection. This page is considered confidential.
Plan of Correction: I removed it during the inspection.
