Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on Tuesday, August 16, 2022 and concluded on Monday, August 22, 2022. During the in person inspection, there were 61 children and 7 staff were present including administrative staff. The children were observed eating lunch, preparing for napping in classroom, and completing bathroom breaks. The sample size was 5 children and 5 staff records. Information gathered during the inspection determined non-compliance with applicable
standards or law and violations were documented on the violation notice issued to the program.

Based on review of staff record, the provider did not ensure subsequent TB screenings are required at least two years from the date of the initial screening.

Evidence: In Staff #2's record, the TB documentation reviewed for the inspection was dated 01/23/19. In Staff#3's record, the TB documentation reviewed was dated 01/21/19.
Plan of Correction: One staff completed TB screening with physician and results received on 8/22.
The staff will schedule appointments for TB screening and results will be reviewed and included in records.

Based on observation and interview, the provider did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence: During the inspection on August 16, 2022, there multiple areas at the program where hazardous substances were observed accessible to children and with the labels "keep out of reach of children", "warning", and "caution".
In the lunch room area, there were bottles of sanitizer on the table nearby the entry.
The hallway closet was observed unlocked containing multiple cleaners, and disinfectants.
The bathroom areas were observed with cleaners and disinfectant sprays on a table and located on shelves.
The classroom area was observed sanitizer on the desk area.
Interview with staff confirmed the storage of hazardous substances at the program.
Plan of Correction: The program has removed hazardous substances and locked them away.
