An unannounced, on-site monitoring inspection was initiated on August 15, 2024 and
completed on August 19, 2024. The on-site inspection began at 8:44 am and ended at 10:17 am. The inspector reviewed compliance in the areas listed above. There were 64 children present and 10 staff. The inspector reviewed 7 children?s records and 5 staff records electronically on August 19, 2024. This inspection included document review, tour of the facility, interviews, and observations

Based on review of documentation and interview, the provider did not ensure to obtain documentation that each person in supervisory position has been certified by practicing physician or physician assistant to be free from any disability which would prevent caring for children under supervision.
