Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on August 15, 2024 and concluded on August 19, 2024. During the in person inspection, there were 64 children and 10 staff were present including administrative staff. The children were observed in chapel, transitioning for snack, completing bathroom breaks/handwashing, rehearsing for upcoming program, engaged in outdoor playground activities, and interacting with their peers and staff. The sample size was 7 children and 5 staff records. Information gathered during the inspection determined non-compliance with applicable
standards or law and violations were documented on the violation notice issued to the program.

Based on staff record and interview, the provider did not ensure to obtain results of all background checks every five years thereafter.

Evidence: In Staff#1's record, the last dated fingerprint based background check was dated September 12, 2018.
In Staff#3's record, the last dated fingerprint based background check was dated January 26, 2018.
In Staff#5's record, the last dated fingerprint based background check was dated January 29, 2019 and central registry results January 29, 2019.
Plan of Correction: The provider will resubmitted background checks for both staff and obtain results. Copies of request will be maintained.

Based on review of staff record and interview, the provider did not ensure to obtain documentation of a tuberculosis screening.

Evidence: In Staff#2's record, there was no documentation of a tuberculosis screening found for review.
Interview with staff confirmed the documentation was not available for review during the inspection.
Plan of Correction: The provider get those forms for the staff by Friday, August 23, 2024.

Based on observations, the provider did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the kitchen area, there were multi-purpose cleaner and deodorizer and dishwasher cleaner observed on the with the labels, "keep out of reach of children" "caution" "warning" accessible to the children in care.
