Inspector Notes

This renewal inspection was initiated by licensing staff and concluded on 07/06/2022, between the hours of 9 am to 1:30 pm. There were 33 children present, ranging from infant to school-age, with 8 staff supervising across 4 classrooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records, and 6 staff records were reviewed.

Information gathered during the inspection determined that there were 2 non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the program.

Standard 22VAC40-185-510-E
Based on review, a medication observed on site was expired and had an expired medication authorization form.

Evidence -
1. On the date of inspection (07/06/2022), the medication on site for Child #1 had an expiration date of 01/13/2022.
2. The Authorization to Administer Medication form completed and signed by the child's parent and doctor had an expiration date of 01/04/2022. Medication had not been administered to Child #1.
Plan of Correction: Medication will be returned to parent. Current medication and authorization will be requested.
Standard 8VAC20-780-160-C
Based on review, 1 of 6 board officer/staff files indicated that it has been more that two years since the date of the most recent tuberculosis (TB) documentation on file at the center.

Evidence -
1. On the date of inspection (07/06/2022), the most recent TB documentation on file for Staff #1 was dated - 01/24/2017.
2. This was confirmed on the date of inspection by Staff #4.
