Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 07/19/2023, between the hours of 10 am to 12 pm. There were 35 children present, ranging from infant to preschool-age, with 8 staff supervising, in 4 separate classrooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records, 3 medications, and 5 staff records were reviewed. A follow up to the last inspection determined that all previous violations have been corrected.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center.

Standard 8VAC20-780-290-A-3
Based on observation, 1 of 8 electrical wall outlets did not have a protective cover.

Evidence -
1. On the date of inspection (07/19/2023), a protective cover to prevent access by the children within the early Montessori toddler room was missing from one electrical wall outlet.
2. There were (7) seven children within the room at the time of observation.
Plan of Correction: To be corrected on the date of inspection by classroom staff.
Standard 8VAC20-780-510-E
Based on review, 1 of 3 medications on site had a signed medication authorization form that had expired.

Evidence -
1. On the date of inspection (07/19/2023), a signed medication authorization form for medication prescribed for Child #6 had expired.
2. The listed dates of authorization was listed as: 09/2022 through 06/2023. Therefore, this authorization expired after 06/30/2023.
Plan of Correction: The medication and authorization will be returned to the parents of Child #6. A new medication authorization will be requested.
