Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 03/15/2024, between the hours of 9 am to 12 pm. There were 79 children present, ranging from infant to school-age, with 14 classroom staff supervising across 7 rooms. The children were engaged in the following: St. Patrick's Day theme crafts, art and story time; center activities within each classroom, infant sleeping, diaper changing, hand washing, large motor skills in the multi-purpose room, outside play, snack, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 10 children files, 3 board officer files, and 10 staff files were reviewed.

Information gathered during the inspection determined that there were (3) three non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center.

Standard 8VAC20-780-160-C
Based on review, 1 of 10 staff files indicated that it has been more than (2) two years from the date of the tuberculosis (TB) documentation on file at the center.

Evidence -
1. On the date of inspection (03/15/2024), the TB documentation on file for Staff #3 had expired.
2. The date of this documentation was - 01/16/2022.
Plan of Correction: Updated TB documentation will be obtained and placed on file.

Based on review, 1 of 10 staff files indicated that the Virginia Department of Education - sponsored orientation course was not completed within 90 calendar days of employment.

Evidence -
1. On the date of inspection (03/15/2024), the Virginia Pre-service training (10 hours) was not completed within 90 days of the start date for Staff #9.
2. The listed start date for Staff #9 was 01/24/2022.
Plan of Correction: This training will be completed and certificate will be placed on file.

Based on review, 1 of 10 children files indicated that a child did not have a separate record that contained all of the required information.

Evidence -
1. On the date of inspection (03/15/2024), a child record was not available for review for Child #9.
2. An enrollment agreement was on file and dated - 02/24/2024.
