Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 03/24/2021 and concluded on 03/25/2021. The director was contacted by telephone to initiate the inspection. There were 17 children present and 4 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the vendor did not ensure that each staff complete annually the Virginia Annual Health and Safety Update Course sponsored by the Department of Social Services as required.

Evidence:
1. The record of staff #1 (DOH:09/25/2001) contained documentation of the Virginia Annual Health and Safety Update dated 04/30/2020.
2. The record of staff #2 (DOH:09/25/2001) contained documentation of the Virginia Annual Health and Safety Update dated 05/24/2020.
3. The vendor acknowledged that the training update was not completed by 03/01/2020 as required.
Plan of Correction: Staff #1 will complete VA Annual Health and Safety Update on time from this incident and forward. Staff #2 same as above.
