Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 12/15/2021 and concluded on 12/20/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 39 children present with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
**New requirements became effective on October 13, 2021. The facility has not yet fully complied with the following requirement: Tuberculosis screening submitted at the time of employment and prior to coming into contact with children. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.
Violations
2Evidence: 1) The immunization documentation in the record for Child #2, enrolled on 10/26/21, was dated 11/05/21. 2) During interview, a member of management reported the immunization records for Child #2 were submitted after the child attended the center.
Evidence: 1) A prescription medication was observed for Child #5 and no written parental authorization was present for this medication. 2) During interview, a member of management reported the written parental authorization for this medication was not present.
