Inspector Notes
An unannounced monitoring inspection was conducted on September 6, 2023 from approximately 9:45 am to 1:15 pm. There were a total of 42 children present, ranging in ages from 15 months to 5 years, with 12 staff supervising.
A total of 5 children records and 5 staff records were reviewed. During the inspection, children were observed engaged in a variety of activities. Staff were observed having positive interactions with the children. The center is equipped with toys and supplies and items were available to the children.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable.
Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use
staff names; list staff by positions only.

The record for Staff #11 (date of hire: 7/17/2013) documented a criminal history check dated
6/27/18 which is beyond 5 years. No additional information was provided.
Evidence:
1. The record of Child #3 (date of enrollment:10/05/22) did not contain documentation of a physical examination.
2. During an interview, the center director acknowledged the record did not document a physical examination.
