The center must request a search of the central registry prior to the employee's first day of employment. 1. Staff #1, employed for two months and 17 days, did not have documentation the central registry was requested prior to employment. The results of the central registry were received 9/16/2025. 2. Staff #3, employed for one month and 20 days, did not have documentation the central registry was requested prior to employment. The results of the central registry were received 10/29/2025. Staff #5, employed for seven months and 18 days, did not have documentation the central registry was requested prior to employment. The results of the central registry were received 03/31/2025.
Plan of Correction: Per the director: We will ensure CRS is complete prior to hire in future.
Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician designee, or an official of the local health department. The record of staff #5: contained a tuberculosis screening dated two months prior to employment.
Plan of Correction: Per the director: Staff will obtain TB screening no more than 30 days prior to hire.

The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. The record of staff #2, employed for four months and 10 days, did not have documentation of completing the Department of Education-sponsored orientation.
Plan of Correction: Per the director: Will have staff complete it asap, I will monitor date due going forward.

Staff members caring for any child for whom emergency medications have been prescribed must have the appropriate Medication Administration Training. Child #6 has an emergency seizure nasal spray medication on site, but there are no staff certified in the Medication Administration Training for Seizure Rescue Nasal Spray Course.
Plan of Correction: Per the director: Staff will complete the seizure rescue nasal spray training course.

When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. The medication authorization for child #7 expired 12 weeks ago. The medication was on site and had not been administered.
Plan of Correction: Per the director: we have asked the parents for an updated authorization.
