Inspector Notes

An unannounced monitoring inspection was conducted May 4, 2023 from 10:40am-12:30pm. There were 8 children directly supervised by 2 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, 2 staff records, 5 children?s records, emergency drills, injury reports, emergency supplies, and policies were inspected. There were reportedly no medications on site at the time of inspection. Children were observed playing outside and learning to make and dye their own yarn. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children. There was an adequate number of CPR/First Aid and Daily Health Observation trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-770-60-C-2
Based on record review, the center did not obtain the results of a Virginia Child Abuse and Neglect Central Registry search for all staff within 30 days of employment.

Evidence: Staff #2 (Date of Hire: 10/28/2022) did not have the results of Virginia Child Abuse and Neglect Central Registry search on file at the time of inspection.
Plan of Correction: Confirmed with DSS that application errors were preventing Staff #2's registry search. Finally received results on 5/19/23.
Standard 8VAC20-780-130-A
REPEAT VIOLATION

Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence: Child #4 (Start Date: 4/28/2023) did not have immunizations on file.
Plan of Correction: Received paperwork from Child #4's parent the day after your visit.
Standard 8VAC20-780-160-A
Based on record review, the center did not obtain documentation from all staff of a negative tuberculosis screening at the time of employment and prior to staff coming into contact with children.

Evidence: Staff #3 (Date of Hire: 10/28/2022) did not have the results of a tuberculosis screening or test on file at the time of inspection.
Plan of Correction: Staff #2 will not be with the N/K class this year. We plan to hire a new assistant in the coming weeks and will ensure he or she completes a tuberculosis screening.

Based on record review, the center did not ensure that all staff have completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence: Staff #1 (Date of Hire: 9/22/2022) and Staff #2 (Date of Hire: 10/28/2022) did not have documentation on file of completing the Virginia Department of Education-sponsored orientation course.
Plan of Correction: Staff #1 completed the orientation on 10/5/22. Staff #2 is no longer in the N/K. We will conduct the orientation with our new hire.
