Inspector Notes

A renewal inspection was initiated on 04/18/2023 and concluded on 04/18/2023. There were 43 children present, ranging in ages from 5 years to 12 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on a review of staff records and interview on 04/18/2023, the center failed to ensure that each staff record reviewed contained a central registry finding within 30 days of employment.
Evidence:
1. The record of Staff #1 (DOH 8/25/2022) did not contain documentation of a central registry finding.
2. The record of Staff #2 (DOH 8/25/2022f) did not contain documentation of a central registry finding.
Plan of Correction: Administration is following up with DSS background checks to find results. If unable to locate the results, then they will resubmit the background checks.
