Inspector Notes

An unannounced monitoring inspection was conducted on June 5, 2023 from 10:00 am until 11:15am. There were 6 children present (19 points) with 2 caregivers supervising. The home, children's records, caregiver records, cribs, evacuation logs, attendance logs, assistant caregiver logs, first kit, flashlights and emergency preparedness procedures were reviewed. The children were observed interacting with caregiver during a story time. Areas of non-compliance were cited on the violation notice. Based on review of children's records, the provider failed to review with the parent the emergency contact information required in 8VAC20-800-60-B-2 to ensure the information is correct.

Evidence:
Child #1 and #2's records do not have documentation of annual review of emergency contact information. Child #1's last annual review is dated 03/06/22 and Child #2's last annual review date is05/28/2021.
Plan of Correction: I will remember to complete the annual reviews.
