Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 4/22/2021 and concluded on 4/27/2021. The director was contacted by telephone to initiate the inspection. There were 5 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, 2 of 2 records did not contain all required information.

Evidence:
1. The proof of identity document in the record for Child #1 did not include the name of the person who reviewed the proof of identity document, and the date the reviewer verified the proof of identity document.
2. The record for Child #2 did not contain the work phone number for each parent.
Plan of Correction: I will add the proof of identity information to the file tonight and I will have the parents complete work information on the form.
