Inspector Notes

An unannounced monitoring inspection was conducted 0n 5/28/2024 from 11:15am-1:30pm. There were 21 children directly supervised by 3 staff. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed finishing lunch and preparing for nap. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 1 of 3 staff did not obtain a tuberculosis screening every two years.

Evidence:
The most recent tuberculosis screening documented in the record for Staff #3 was dated 5/26/2022.
Plan of Correction: An appointment will be scheduled by the end of the week.
Standard 8VAC20-780-260-A
Based on record review, an annual fire inspection report from the appropriate fire official having jurisdiction was not obtained annually.

Evidence:
The most current documentation of a fire inspection was dated 3/21/2023.
Plan of Correction: The center will call to request an inspection.

Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
Potting soil was observed in the grassy area in the playground accessible to children.
Plan of Correction: The potting soil was removed from the playground during inspection.

Based on observation, fall zones did not encompass sufficient area to include the child?s trajectory in the event of a fall.

Evidence:
1.The fall zone for one outdoor play structure that includes a slide did not contain a fall zone of 6 feet around all sides as required. When facing the slide, the distance from the slide to the fence measured 5 feet, 2 inches.
Plan of Correction: The playground equipment will not be in use.

Based on record review, 1 injury report reviewed did not contain all required information.

Evidence:
1. The injury report for Child #5 did not contain the following required information:
time of the injury , time when parents were notified, documentation on how parent was notified, any future actions to prevent recurrence of the injury.
2. The information provided for type and circumstance of the injury only stated "right face".
Plan of Correction: The center will ensure all information is included when completing injury reports.

Based on record review, 1 of 4 children?s records did not contain all required information.

Evidence:
1.The record for Child #2 did not contain documentation of the name address and telephone number of 2 individuals to contact in an emergency if the parents cannot be reached.

2.The information for the parents was listed on the documentation requesting emergency contacts.
Plan of Correction: Emergency contact information will be requested from the parents.
