Inspector Notes
An unannounced monitoring inspection was conducted 0n 5/28/2024 from 11:15am-1:30pm. There were 21 children directly supervised by 3 staff. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed finishing lunch and preparing for nap. Areas of non-compliance are identified in the Violation Notice.

6Evidence:
The most recent tuberculosis screening documented in the record for Staff #3 was dated 5/26/2022.
Evidence:
The most current documentation of a fire inspection was dated 3/21/2023.
Evidence:
Potting soil was observed in the grassy area in the playground accessible to children.
Evidence:
1.The fall zone for one outdoor play structure that includes a slide did not contain a fall zone of 6 feet around all sides as required. When facing the slide, the distance from the slide to the fence measured 5 feet, 2 inches.
Evidence:
1. The injury report for Child #5 did not contain the following required information:
time of the injury , time when parents were notified, documentation on how parent was notified, any future actions to prevent recurrence of the injury.
2. The information provided for type and circumstance of the injury only stated "right face".
Evidence:
1.The record for Child #2 did not contain documentation of the name address and telephone number of 2 individuals to contact in an emergency if the parents cannot be reached.
2.The information for the parents was listed on the documentation requesting emergency contacts.
