During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 4/23/2025 and completed on 4/23/2025, as a part of the licensure period. The on-site inspection began at 10:18am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were twenty children present and four staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/30/2025. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION
The center must ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

Two sections of the wooden playground border surrounding the mulch had deteriorating wood with sharp protruding splinters. All six swings had rusting chains and the merry-go-round had rust on all seven handlebars, gripped by children on the playground. Child A's infant crib had a two inch dried stain that had not been cleaned in daily sanitation routines in addition to a three inch loose plastic label. The infant room's diaper changing table had areas of peeling paint measuring about three inches. The Toddler classroom's floor mat had two torn and loose sections measuring one inch. The Toddler classroom's wooden bookshelf had three shelves with chipped and peeling paint, affecting an area of approximately three feet.
Plan of Correction: Handyman will be repainting areas that needed paint and fix the playground
New bookshelf was brought in and mat was replaced.

REPEAT VIOLATION:
Hazardous substances must be in a locked place that prevents access by children.

In the infant classroom, the sanitizing solution was in an unlocked cabinet, out of reach of children.
Plan of Correction: Cleaning supplies was removed and put in lock cabinet during inspection.

Cleaning and sanitizing materials must not be located above food and must be stored in areas physically separate from food.

The cleaning and sanitizing material was stored above a bottle of baby puffs and a bag of goldfish.
Plan of Correction: Cleaning supplies and food container were removed during inspection.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing must comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard.

The resilient surfacing under the slides, climbing ladder and around the merry-go-round had compacted mulch measuring only one inch in depth, which is five inches below the required standard.
Plan of Correction: Some resilient surfacing was pushed back to the front of the playground equipment during inspection and more mulch is scheduled to arrive by 5/6/25.

Toys or objects hung over an infant in a crib that are strung across the crib may not be used for infants over five months of age or infants who are able to push up on their hands and knees.

Child A's crib had a baby Einstein musical mobile and a toy activity arch attached and arched across the crib.
Plan of Correction: Toys were removed from the crib during inspection.
