Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A monitoring inspection was initiated on 12/09/2021 and concluded on 12/10/2021. The Director was contacted by telephone and a virtual inspection was conducted. There were 24 children present, ranging in ages from 18months to 4years, with 6 staff supervising. The Inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of 2 children's records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and one violation was documented on the violation notice issued to the program.

Based on a review of staff health records, it was determined that at least every two years from the date of the first initial screening or testing, all staff members did not obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
The TB screening for staff person A expired on 12/13/2019.
Plan of Correction: She made an appointment for next Monday.
