During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on November 6, 2024. The on-site inspection began at 10:00am and ended at 12:11pm. The inspector reviewed compliance in the areas listed above. There were 63 children present and 18 staff. The inspector reviewed 8 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-40-J
Injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services.

The injury prevention procedures were last updated in 2022.
Plan of Correction: I will review the procedures and update as needed today.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child #1 and Child #6, enrolled for 2 weeks, was missing the following required components: names of persons authorized to pick up the child, name and phone number of child's physician, and the previous child day care attended by the child.
Plan of Correction: I will have the parents complete the information on the application when they pick up their children today.
