During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on April 24, 2025. The on-site inspection began at 10:05am and ended at 12:10pm. The inspector reviewed compliance in the areas listed above. There were 68 children present and 12 staff. The inspector reviewed six children?s records and seven staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. The records for Child #1 did not have documentation of additional immunizations. The last documentation is dated ten months ago.
Plan of Correction: We will contact the parents today that we need the updated immunization record.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Cleaning products and sanitizing solutions were in unlocked cabinets, within reach of children in classroom#1 and #2.
Plan of Correction: Teachers will be reminded to keep the cabinets locked, and ensure cabinets are locked immediately after returning the products to the cabinets. Cabinets were locked during the inspection.
The violation was corrected during the inspection.
Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. There was no allergy list posted in classroom #1 and #3.
