Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on March 25, 2021 and concluded on March 26, 2021. The Director was contacted by telephone to initiate the inspection. There were 34 children present and 6 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on a review of records, the provider did not ensure all caregivers annually complete the Virginia Annual Health and Safety course, sponsored by the Department of Social Services.
Evidence: 1. The records of Staff #1 (DOH 4/5/02) contained documentation of the 3-hour preservice training dated 3/25/21.
2. The records of Staff #2 (DOH 4/8/13, and Staff #3 (DOH 3/20/12) each contained documentation of the 3-hour preservice training dated 3/24/21.
Plan of Correction: Make sure that staff complete Health and Safety updates yearly by the end of February.
