Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated on October 8, 2021 and concluded on October 12, 2021. There were 30 children present with 8 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on a review of records and interview, the vendor did not obtain current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of the children in care within 90 days of employment.
Evidence: 1. The record of Staff #2 (DOH 7/5/19) did not contain documentation of certification in CPR. 2. Administration confirmed Staff #2 did not have CPR training.
Plan of Correction: Will ensure CPR training within 90 days of employment.

Based on a review of records and interview, the vendor did not ensure each staff member had a current certification in first aid within 90 days of employment.
Evidence: 1. The record of Staff #2 (DOH 7/5/19) did not contain documentation of certification in first aid 2. Administration confirmed Staff #2 did not have first aid training.
Plan of Correction: Will ensure first aid certification training within 90 days of employment.

Based on a review of records, the vendor did not ensure that each staff complete the Virginia Annual Health and Safety Update Training by March 1, 2020, and annually thereafter.
Evidence: The record of Staff #3 (DOH 12/27/12) contained documentation of Preservice Training dated 3/29/21.
Plan of Correction: Annual Health and Safety training for each employee before March 1st each year.
