Inspector Notes

An unannounced monitoring inspection was conducted on Monday, November 6, 2023 with the center director. There were 51 children present, ranging in ages from 6 months to 5 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center did not ensure to maintain a complete written record of children's serious and minor injuries in which entries are made the day of occurrence.

Evidence:
1. 3 injury reports were reviewed. 2 of the injury reports were missing the future prevent recurrence, and 2 were missing the time parent was notified.
