Inspector Notes

An unannounced inspection was initiated and completed on June 24, 2024, in response to a complaint received by the licensing office on June 12, 2024 relating to allegations of forbidden actions and behavior guidance. The inspector reviewed 1 child?s record on-site. This inspection included interviews, observations, and document review.

The preponderance of evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Based on interview, the center did not ensure that the forbidden action of enclosure in a small space where a child could not freely exit themselves was forbidden as required.

Evidence:
1. In interview, staff stated that on 6/6/2024 child #1 (age 3) was confined to a highchair for approximately 10 minutes because he would not stay seated during a movie. In the interview, witness #1 stated that child #1 was the only child confined to a chair while the other children were observed sitting freely.
2. Staff acknowledged that the highchair was not being utilized for its intended purpose when child #1 was placed in the chair. They acknowledged that child #1 could not freely exit without assistance.
