Inspector Notes

An unannounced monitoring inspection was conducted April 26, 2024 between the hours of 9:30am - 11:05am. There were 10 children (26 points) directly supervised by 3 caregivers. The home, 3 caregiver records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely and having positive interactions with the caregivers. There was an assortment of age-appropriate materials for children. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within five business days in order for it to be included on the public website.

Based on record review, the provider did not obtain for each adult household member, a current Report of Tuberculosis (TB) Screening form, every two years from the date of the first screening.
Evidence:
1) Household member #4's most recent TB screening on file expired on 11/26/2021. A subsequent TB screening was not obtained every two years.
