During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 4/07/2026. The on-site inspection began at 9:19am and ended at 11:20am. The inspector reviewed compliance in the areas listed above. There were 72 children present and 11 staff. The inspector reviewed 7 children?s records and 7 staff records on-site on 4/07/2026. This inspection included document review, tour of the facility, and observations.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
5There was no physical examination in the record of child #7, who had been enrolled for approximately one year and eight months.
In the Class A, the bathroom children use had a mop with cleaning solution in it accessible to children and not locked.
In the Toddler Class, a wood care polish, sanitizer and white out were on a high shelf that was not locked. Another bottle of sanitizer was on a shelf near the light switch by the door.
Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually.
The information in the record of child #7 had not been updated in over one year and eight months which is overdue by eight months.
The Center failed to implement a monthly practice evacuation drill the following months:
December 2025, January 2026 and February 2026.
Each center shall maintain and keep at the center a separate record for each child enrolled
that contains all of the required information.
The record for Child #7, enrolled for approximately one year and eight months, was missing the following required components: parent's work phone numbers.
