Inspector Notes

An unannounced mandated inspection was completed on 4/15/24 from 10:15am until 11:40. There were 14 children and 3 staff present. Five children?s records were reviewed. Children were observed playing outside, at centers, washing hands and coloring.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.

Information gathered during the inspection determined non-compliance with applicable standards or law and violation (s) were documented on the violation notice issued to the program.

Based on observation and measurement, the required resilient surfacing was not maintained.
Evidence:
Mulch measured 3-4 inches around the two blue slides and around the blue climbing section; the required depth is 6 inches.
Plan of Correction: Mulch will be redistributed to maintain the required depth of mulch.
