Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on 10/22/25 between 11:15 and 12:50. The inspector reviewed compliance in the areas listed above. There were 3 children and 3 staff present. The inspector reviewed 3 children?s records and 1 new staff record electronically. This inspection included document review, tour of the facility, interviews, and observation.

Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #1, who had been employed several months, did not have a completed national criminal background check.
Plan of Correction: Staff had completed the fingerprint check, but the results might have been lost in the flood. HR has contacted the office of background check for a copy,

Where playground equipment is provided, resilient surfacing must meet the requirement of 6 inches in the use zone.

The rubber mulch measured between 2-4 inches around slides and climbing wall.
Plan of Correction: Staff have requested a rake so that mulch can be maintained at the required depth.

Ground supports shall be covered with materials that protect children from injury.

There was concrete showing through the mulch under the blue climbing wall.
Plan of Correction: Maintenance will be contacted to provide a cover of the ground support.
