Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on 10/22/2025. The on-site inspection began at 11:15 and ended at 1:15. The inspector reviewed compliance in the areas listed above. There were 10 children and 3 staff present. The inspector reviewed three children?s records. This inspection included document review, tour of the facility, interviews, and observation.

Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record for child # 3 and they had been enrolled for 3 months.
Plan of Correction: The parents will be notified to a physical examination of the child.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing must comply with the resilient surfacing requirement of 6 inches.

The rock climbing wall and slides had 2-4 inches of mulch in the fall zone.
Plan of Correction: Staff will get a rack to redistribute the mulch to the required depths.

Ground supports shall be covered with materials that protect children from injury.

There was a ground support of concrete showing underneath the rock climbing wall.
Plan of Correction: Maintenance will be contacted to cover the concrete.
