Inspector Notes

A renewal inspection was initiated on October 4, 2022 and concluded on October 12, 2022. There were sixty-eight children present with ten staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and six staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review, facility did not ensure 1 of 5 staff records has required out of state criminal history check prior to the individual beginning employment when the individual has lived outside the state of Virginia within the past 5 years.

Evidence: The record for staff #5 (start date 6/27/2022) had documentation of living in 2 states outside the state of Virginia within the past 5 years and there was not a completed criminal history search for each of those states prior to the staff beginning employment.
Plan of Correction: One criminal history search was completed on 10/12/2022. One criminal history search request was submitted on 10/12/2022. In the future, out of state criminal history searches required will be completed prior to hiring the individual.
