Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due the pandemic.

A renewal inspection was initiated on 1/25/22 and concluded on 1/27/22. The director was contacted by telephone to initiate the virtual inspection. There were 57 children with 14 staff. The children were engaged in a variety of activities to include circle time, large motor activities in the in-door playroom and free play.

The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children and 3 staff files to ensure documentation was complete.
The information gathered during the inspection determined violations with applicable standards or law.

Please send me the sex offender background check for agent of the business entity by 2/3/22.

Based on interview, it was determined that an agent of the business entity did not have an out of state sex offender name search for Florida. This background check search was required by 9/30/20.
Plan of Correction: Upon receipt/completion I will send this background check.
Standard 8VAC20-780-260-B
Based on interview and a review of the record keeping, it was determined that the center failed to obtain an annual approval from the health department.
Evidence:
The health inspection report is dated 9/30/20.
Plan of Correction: This week I reached out to the health department regarding the need for an inspection. Due to COvid 19 the health department is unable to conduct the inspection at this time. I will send licensing a copy of the health inspection upon completion.
Standard 8VAC20-780-550-D
Based on interview and a review of the escape drill log, it was determined that the center is not conducting monthly evacuation drills.
Evidence:
The center did not conduct escape drills in April, May, June, July, August and October 2021.
Plan of Correction: We will make sure to conduct and document monthly escape drills.
