Inspector Notes

An unannounced monitoring inspection was conducted today with the director from 1:20 pm to 3:25 pm. Fifty one children were present with seven staff providing supervision. The ages of the children ranged from 5 months to 5 years. Most of the children were napping during the walk through of the building.
Six children and four staff files were reviewed as well as medications for two children.
Send a copy of the health inspection once completed. The most recent fire inspection is dated 10/7/22. The playground was not inspected today due to the inclement weather.

Standard 8VAC20-780-140-A
Based on a review of six children's files and interview, it was determined that the child A first attended the center on 10/19/22 and there was no physical on file within 30 days after the first day of attendance.
Plan of Correction: I will get the physical.
Standard 8VAC20-780-260-B
Based on a review of recordkeeping and interview, it was determined that there has been no annual inspection from the health department.
Evidence:
The health inspection is dated 9/30/20.
Plan of Correction: I will reach out to the health department and have them schedule an inspection as soon as possible.
Standard 8VAC20-780-270-A
Based on observation, it was determined that the center is not maintained in a safe condition.
Evidence:
The Licensing Inspector (LI) observed a fan on top of the cabinet in the Twos A classroom. The cord to the fan was plugged into the outlet and hung in reach of the children, which presented a hazard of a child pulling on the cord and the fan falling on top of a child(ren).
Plan of Correction: The fan was immediately removed and no longer presents a hazard. We will talk with staff about the importance of maintaining a safe environment.
Standard 8VAC20-780-550-F
Based on interview and a review of recordkeeping, it was determined that the center has not practiced lockdown procedures at least annually.
Evidence:
The last documented lockdown drill is dated 2/26/18.
Plan of Correction: We will conduct a lockdown drill this month.
Standard 8VAC20-780-80-A
Based on observation and interview, it was determined that the center is not maintaining a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
Two children were listed on the attendance log, however three children were present in the toddler A classroom.
Plan of Correction: I will add this newly enrolled child to the attendance roster. We will make sure to maintain up to date attendance.
