During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 10/17/2024 in response to a complaint received by the licensing office on 09/23/2024 relating to allegations of supervision.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/08/2024. A POC submitted after this date will not appear on the public website.

The required ratio for children ages two years is one staff per eight children.
1. Staff A was supervising 9 to 12 children (youngest child two years old) in the Pre-School two Room from approximately 6:37am-7:00am.
2. Staff B was observed supervising 13 children (youngest child two years old) in the Pre-School 1-A Room at 8:45 a.m.
Plan of Correction: Staff were retrained on proper ratios per age group and the importance of maintaining ratios at all times. Staff involved received documentation on our zero-tolerance policy on child supervision and have been asked to re-read CCEI Training Course ID COA Zero; Course name COA Zero Tolerance to be completed by 11/04/2024. Management also adjusted the schedule to allow for more coverage in the morning from 6am - 7am as well as hired 3 additional staff members.
