Inspector Notes
An unannounced monitoring inspection was initiated and completed on 09/09/2025 in response to a self-report received by the licensing office on 09/08/2025 relating to supervision. The inspector reviewed one children?s record and one staff record on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/03/2025. A POC submitted after this date will not appear on the public website.
Violations
1On 9/04/2025, child A (Age 2 years) was receiving a diaper change by Staff A on a mobile changing table in the bathroom of the two year old classroom. Child A was in the seated position on the changing table and turned to retrieve a diaper wipe. While turning, the child pushed their hands on the wall which caused the changing table to move. The wheels on the changing table were not in the locked position. Child A subsequently fell between the changing table and the wall, hitting their forehead on the floor. Child A sustained a bump to the right side of their forehead. Child A was later taken to the emergency room for evaluation.
