During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was initiated and completed on 09/09/2025 in response to a self-report received by the licensing office on 09/08/2025 relating to supervision. The inspector reviewed one children?s record and one staff record on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their, care, protection, and guidance.

On 9/04/2025, child A (Age 2 years) was receiving a diaper change by Staff A on a mobile changing table in the bathroom of the two year old classroom. Child A was in the seated position on the changing table and turned to retrieve a diaper wipe. While turning, the child pushed their hands on the wall which caused the changing table to move. The wheels on the changing table were not in the locked position. Child A subsequently fell between the changing table and the wall, hitting their forehead on the floor. Child A sustained a bump to the right side of their forehead. Child A was later taken to the emergency room for evaluation.
Plan of Correction: 9/5/2025 the changing table was removed from the classroom and 9/9/25 was thrown into the dumpster. Moving forward, management and staff will ensure that the teacher utilizes the stationary changing table in the classroom.
