Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in-person component tour of the program. A monitoring inspection was conducted on 01/05/2022. There were 67 children present and 6 staff. The The Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation the center did not ensure all areas of the center were maintained in a safe condition.
Evidence:
1. The ceiling in the building had water stains and some of the tiles cracked/broken.
2. The door between the reception area and main daycare had peeling paint and had chipped wood at the bottom of the frame.
3. Next to the infant room there was about a 2inche hole in the wall, exposing peeling paint and drywall.
4. The ceiling vents had rust and black spots on them.
5. The tank for the turtle in the reception area had an odor and the water in the tubes were green and murky.
