Inspector Notes
A monitoring inspection was initiated on 3/19/2024 and concluded on 4/23/2024 in response to a self-reported incident regarding unacceptable behavior management techniques used by staff. The investigation included review of video footage, pictures and interviews. The inspector reviewed compliance in the areas of administration, staffing and supervision, and programming.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based upon interview, the facility has not ensured that when staff are supervising children, they ensure their care and protection.
Evidence:
Interview revealed that staff took a picture of staff 1 using unacceptable discipline techniques when interacting with a child and posted the picture to the parent portal. This was done instead of intervening and reporting the unacceptable action to management or authorities.

Based upon staff interview, the facility has not ensured that in order to promote the child's emotional and social well-being and growth, staff interact with the child to provided needed help, comfort and support.
Evidence:
Staff 1 was described by staff as interacting with children using a "harsh", "stern" and "sharp" tone of voice when children misbehave, "alarming" both the children and staff.

Based upon video footage, photos and staff interview, the facility has not ensured that the action of staff using physical punishment such as roughly handling a child is forbidden.
Evidence:
1. Video footage showed staff 1 roughly taking a two year old child by the chin, holding the child's head while speaking with the child.
2. Video footage showed staff 1 grabbing a two year old child by the upper arm and pulling the child away from another child.
3.Photo evidence showed bruising on the child's upper arm indicative of an adult grabbing the arm.
3. Staff interviews confirmed the rough handling of the two year old child to include grabbing the child by the chin and grabbing the child by the upper arm.
