Inspector Notes
An unannounced inspection was initiated on 06/21/2024 and completed on 07/29/2024 in response to complaints received by the licensing office on 05/08/2024, 06/03/2024 and 06/10/2024 relating to allegations of administration, staffing, supervision, physical plant, programs and special services. This investigation included:
? document review,
? interviews, and
? observations
The preponderance of evidence gathered during the investigation supports the allegations of programs/behavior guidance; therefore, the complaint is determined to be valid. Information gathered during the inspections determined non- compliance with applicable standards or law and a violation is documented on the violation notice issued to the program.
Though not a part of the original complaint, a violation that was not reported were found related to administration and is documented on the violation notice issued to the program and reviewed with the program administrator on 07/29/2024.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today 07/29/2024, which will be the close of business on 08/05/2024. A POC submitted after this date will not appear on the public website.
Violations
2Evidence:
After twice having requested a copy of the facility?s written staff policies as required by the Standards, the facility was unable to provide the written staff policies as indicated below:
- Procedures for supervising a child after activities and field trips have begun
- Procedures to confirm the absence of a child when the child is scheduled to to arrive from another program
- Procedures for identifying where children are at all times, including before leaving for a field trip site and upon return to the center
- Procedures for action in case of lost or missing children
- Policy for the administration of medication
- Emergency evacuation, relocation, shelter-in-place and lockdown procedures, and
- Precautions in transporting children.
Evidence:
1. Interviews revealed that staff 1 does not ?support the social and emotional growth of the children? . Described interaction of Staff 1 with children included:
- a child had a toileting accident and staff 1 called the child a baby and stated that ?boys don?t cry? .
- staff 1 calls children a ?cry baby?.
- staff 1 was described as ?talking down to kids?, making statements such as ?why are you acting this way?? and ?why can?t you understand??.
- staff 1 ?makes children cry?
- staff 1 uses inappropriate discipline practices such as ?not letting children play together for a whole week?.
- staff 1 makes statements to the children such as ?so and so can?t listen?
- staff 1 ?embarrasses? children by making statements such as ?let?s not do what so and so did?.
- Staff 1 ?talks over? the children.
2. Interviews revealed that staff 1 does not interact with staff to provide needed help and support.
- Staff 1 was described as being ?hard to approach?.
- Staff 1 ?speaks inappropriately to staff.
- Staff stated that they limit interactions with her staff 1.
