Upon arrival, all lights in the Starfish classroom were off except a small table lamp and the far bathroom light. Children were running around the classroom, climbing on each other's cots, and climbing on shelving units. Children and staff could not adequately see each other. Staff did not provide activities or guidance as one staff person was changing diapers and the second staff person was walking around the room.
During nap time, all lights were off in the Sea Turtles room except a small red lamp. The hallway windows were covered by blankets. It was too dark for staff to adequately observe the children on their cots to provide assistance, if needed.

In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Such a program leader shall supervise no more than two aides.
Interview revealed that staff 1 has been on duty by herself in the two year old classroom from 8:00 AM until 9:00 AM for the past two months and in the afternoon when staff 2 moves into the office. Staff 1 is not program leader qualified.

The staff to child ratio for children ages 16 months - 24 months required whenever children are in care is one staff person for every five children.

On 10/27/25, administration was informed that children are not permitted to transition to the next age group within 30 days of their upcoming birthday. This child was transitioned on 9/8/2025, prior to the administration being aware of this rule.

The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Child 1 sustained an injury requiring outside medical attention on 10/27/2025. The facility did not inform the department's representative until 11/13/2025 when it was revealed during investigation and the facility was instructed to file the report.
