Inspector Notes
A renewal inspection was conducted on 10/27/2022 between the hours of approximately 9:15 am to 10:45 am. There were 12 children present, ranging in ages from 5 months to 3 years, with 3 staff supervising. At the time of entrance, older children were observed during circle time with the Provider: they reviewed colors and shapes, sang the ABC and the Hockey Pockey songs. After that, the provider offered puzzles to the children. Four infants were observed in their cribs.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There were no medications on site today. A total of 12 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
6Evidence:
1) At the time of entrance, the inspector observed 12 children in care in the fdh. The attendance record had documented that only 11 children were in care.
2) Upon review of the attendance record and all children's records, the inspector noted that two of the children listed in the attendance list did not have records in the fdh. After inquiring the provider about it, she stated that they did not attend the fdh. The provider proceeded to update the attendance with the names of two children that were present but not included in the attendance list.
Evidence:
1) The inspector observed a child sitting on a high chair after having finished eating breakfast. Upon inspection, the licensing inspector noted that the safety strap of the high chair was not fastened to secure the child.

1) The record for Child #1 did not contain the telephone number of each custodial parent's place of employment.
Evidence:
1) On the date of inspection, a battery-operated weather band radio was available in the family day home, however, the batteries did not work and there were not enough additional batteries available to make it work.

1) The most recent review of the emergency information in the record for Child #3 was dated 07/09/2021, for Child #4 was dated 08/04/2021, for Child #5 was dated 07/07/2021, for Child #6 was dated 10/08/2021.
