Inspector Notes

A monitoring inspection was conducted on 05/02/2023 between the hours of approximately 10:00 and 11:00 am. There were 12 children present, ranging in ages from 11 months to 3 years, with 2 caregivers supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the Provider did not ensure that each child have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence:
1) The record for Child #1 (start date 09/30/2019) did not contain the results of a physical examination.
Plan of Correction: Child's pghysical examination was obtained and added to the record file.
