Inspector Notes

A complaint inspection was conducted on 07/26/2023 to investigate a complaint received by the department involving the area of forbidden actions and supervision. During the investigation, interviews were conducted with staff, and staff records were reviewed.
The evidence gathered during the investigation have determined that the validity of allegations in the complaint could not be substantiated. In addition to the original complaint, there were also violations found during the inspection regarding records. See the violation notice for more details.

Standard 8VAC20-800-140-D
Based on interview with the provider and record review, the provider did not ensure that the substitute provider record and sign the time of arrivals and departures on each day that the substitute provider works.
Evidence:
1) On the date of inspection, the provider stated that a substitute provider has covered for her when needed. When the LI requested to see the substitute provider?s attendance record, the provider stated that they have not kept an attendance record for the substitute provider.
Plan of Correction: Substitute provider attendance record with time of arrival and departure was updated on 07/06/2023.
Standard 8VAC20-800-50-A
Based on document review, the family day home did not keep a written record of children in attendance each day.
Evidence:
1) On the date of inspection, an attendance record for the current week was not available for review.
Plan of Correction: Attendance record was updated on 07/06/23.
Standard 8VAC20-800-60-A
Based on record review, the provider did not maintain an up-to-date record at the family day home for each child.
Evidence:
1) On the date of inspection, no record was available for review for two of the children in attendance that day.
Plan of Correction: The two missing records for two children were updated on 07/06/2023.
