Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on September 18, 2024, as a part of the licensure period. The on-site inspection began at 11:15am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 8 children present, 1 provider and 2 caregivers; the point total was 24. The inspector reviewed 5 children?s records, 1 provider, 2 caregivers records onsite. This inspection included document review, tour of the home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/2/2024. A POC submitted after this date will not appear on the public website.
Violations
4In addition to satisfactory completion of first aid training and CPR training caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
The records for caregiver #1 and caregiver #2 were missing a minimum of sixteen clock hours of additional annual training. Caregiver #1 obtained 10 hours and caregiver #2 obtained 9.5 hours of additional training.
Child #5 was missing the employment information for both parents. The child was present during the inspection and had been in care for a month.
The emergency plan was not reviewed and was overdue by one year.
The immunization record was obtained after Child #5?s first date of attendance. The child was present during the inspection and had been in care for one day.
